Hemodynamic Stability Patients’ responses to both mechanical and spontaneous ventilation vary according to myocardial function and reserve capacity. Cardiac failure, which may not be apparent when a patient is receiving mechanical ventilation, can be exacerbated with transition to spontaneous ventilation. Signs of hemodynamic instability include an acute or gradual decrease in arterial blood pressure, tachycardia, bradycardia, dysrhythmias, weak peripheral pulses, decreased pulse pressure, acute or gradual increase in pulmonary capillary wedge pressure, decreased cardiac output, and decreased mixed venous oxygen saturation. Hemodynamic stability is difficult to evaluate when a patient is receiving vasoactive support, a common situation after cardiac surgery. Therefore, during weaning, hemodynamic parameters should be monitored closely in such patients.

Weaning from mechanical ventilation

INTRODUCTION This article reviews the literature on the weaning process in relation to nursing care. Weaning is defined as the process of gradually reducing mechanical ventilatory support as the patient’s own respiratory system recovers from disease (Keen, 2000). This process has been identified as one of the highest-ranked research priorities in nursing care by the American Association of Crit...

Comparison of the effect of pressure support ventilation and volume assured pressure support ventilation on weaning patients off mechanical ventilation after cardiac surgery

Background: Weaning from mechanical ventilation in the intensive care unit is an important step in the treatment process. More patients in less than 6 hours after cardiac surgery can be separated from mechanical ventilation and extubated. However, 20-40% of patients after cardiac surgery, due to dysfunctional ventilator weaning response (DVWR), still remain under mechanical ventilation. Therefo...

The effect of protocol of weaning from mechanical ventilation on gastrointestinal function in people undergoing coronary artery bypass graft surgery (CABG)

Abstract Aim. The aim of this study was to examine the effect of protocol of weaning from mechanical ventilation on the gastrointestinal function in people undergoing CABG. Background. The most common problems after cardiac surgery in the intensive care unit are change in vital signs, respiratory problems, unsuccessful separation from mechanical ventilation, and gastrointestinal problems. Due t...

Prolonged weaning: from the intensive care unit to home.

Weaning is the process of withdrawing mechanical ventilation which starts with the first spontaneous breathing trial (SBT). Based on the degree of difficulty and duration, weaning is classified as simple, difficult and prolonged. Prolonged weaning, which includes patients who fail 3 SBTs or are still on mechanical ventilation 7 days after the first SBT, affects a relatively small fraction of me...

حسابرسی مراقبت‌های پرستاری مرتبط با جداسازی نوزادان از دستگاه تهویه مکانیکی در بخش‌های مراقبت‌های ویژه نوزادان

Background & Aim: Mechanical ventilation is one of the most common treatments in neonatal intensive care unit and is however associated with many complications. One of the ways to reduce complications is providing nursing care according to standards. This study was conducted with the aim of determining the conformity of nursing care related to weaning neonates from mechanical ventilation to the...

Weaning older patients from long-term mechanical ventilation: a pilot study.

BACKGROUND As older persons in the intensive care unit increasingly require long-term mechanical ventilation, accurate indications of readiness for weaning from ventilatory support are needed to avoid premature extubation. OBJECTIVE To describe temporal changes in pulmonary and systemic variables in older adults receiving long-term mechanical ventilation. METHODS After 3 days of unsuccessfu...
